Appendicitis is always on differential when it comes to a patient with an acute abdomen and is also one of the most common reasons for emergent abdominal surgeries.   There is a rich history in trying to diagnose appendicitis, especially in the time before advanced imaging, and many of classic findings in appendicitis stem from these early years of medicine.  In this episode, we will cover the pathogenesis, historical clues, physical exam findings, pretest probability scoring systems, radiographical considerations, and management of one of the oldest surgical diseases in medicine.
